Intake Process
- Call from Case manager, guardian or potential client requesting information
- Intake packet sent to Case Manager, guardian or potential client
- Intake Evaluation received by Independent Haven; reviewed by Program Manager
- Independent Haven requests collateral information
- Collateral information received by Independent Haven
- Meet and Greet scheduled; usually held at Independent Haven’s Main Office
- Nursing Assessment with Independent Haven’s R.N. Manager & site Residential Supervisor
- Site Visit
- Doctor’s Admission Orders and Physical, and mantoux received by Independent Haven
- Admission meeting, housing meeting and move-in
20 Hours Per Week of Constructive Activity
Through experience, Independent Haven has found clients are more successful when they are involved in at least 20 hours per week of constructive activity. We can assist clients in meeting the 20 hour community-based activity requirement through a variety of resources including employment, day treatment, CD treatment, therapy, educational programs, volunteering or a combination of activities.
