Customized Services at Independent Haven

Independent Haven offers a continuum of customized services.  Below is an overview of the services we offer;

Service Including
24 hour On-Call Registered Nurse Oversight of Medication

As Needed (PRN) Medication

Serious Illness, Injury

Health Related 911 calls

Medical or Urgent Care

24 hour On-Call Residential Supervisor Resource to direct support staff

Reporting of suspected abuse, neglect financial exploitation

Mental health and crisis stabilization

Mental Health Stabilization Weekly Check-in with Supervisor

Daily Check-in with direct support staff

Communication with other providers

On-Site Asleep Overnight Staff Emergency Assistance

PRN Medication Administration

Activities On-Site activities such as bingo, movie night, Wii, computer lab, grilling, walks, games

Off-Site activities such as YMCA, bowling, movies, MOA

All Independent Haven activities such as Camp Confidence, Exploring Minnesota, ILS Classes

Medication Administration Based on nursing assessment and doctor’s orders, client services may include:

-Client sets-up and orders medications, RN supervise processes

-Client sets-up medications, RN supervises; client does not call in when taking meds, staff orders meds

-Client sets-up medication, RN supervises; client calls in when taking meds, staff orders meds

-RN sets-up medi- planner/med-machine; client does not call in when taking meds. Staff orders meds

-RN sets-up medi-planner/med machine; client calls in when taking meds, staff orders meds

-Staff administer and order medications

Appointment Scheduling

 

One (or a combination) of the following:

-Verbal Prompts/Reminders = Staff prompt client to make and attend appointments

-Visual oversight = Staff observe client making their own appointments and complete and send Provider Communication Forms

-Side by side =  Staff provide names, numbers, calendars, and written reminders to client while client makes appointment; staff complete and send Provider Communication Forms to providers

-Totally Dependent = Staff make all appointments; staff complete and send Provider Communication Forms to providers

Transportation Scheduling

 

One (or a combination) of the following:

-Verbal Prompts/Reminders = staff remind client to make and cancel rides, client notifies staff when rides are made

-Visual oversight = staff observe client arranging transportation

-Side by side assistance = staff schedule rides with client present

-Totally Dependent = staff schedule transportation without client involvement

Money Management

 

One of the following:

-Independent = client pays own bills, may be own rep-payee, budgets own money without staff involvement

-Side by side assistance = client has rep-payee, staff assist with budgeting, paying bills, money disbursement.

Nutrition Support:  Menu Planning

 

One (or a combination) of the following:

-Verbal prompts and reminders = staff remind client to make list and menu plan

-Visual oversight = staff review client prepared menu plan and grocery list

-Side by side assistance = staff develop grocery list and menu plan with client

Nutrition Support:  Grocery Shopping

 

One (or a combination) of the following:

-Verbal prompts and reminders = staff remind client to go grocery shopping

-Visual oversight = staff accompany client to grocery store

-Hands-on assistance = staff accompany client to grocery store, give feedback on food choices, assist client with budgeting, assist with carrying groceries

Nutrition Support:  Food Prep One (or a combination) of the following:

-Verbal prompts and reminders = staff remind client to prepare meals by following menu

-Visual oversight = staff present in client’s apartment while client is cooking, staff may be present while client eats

– Side by side assistance = staff assist client with meal prep

Social – Interpersonal Skills

 

One (or a combination) of the following:

-Verbal prompts and reminders = staff use positive verbal reinforcements and proactive verbal intervention techniques

-Visual oversight = staff use reactive intervention techniques

– Side by side assistance = staff need to administer PRNs, RS/RN intervention

Personal Hygiene – -Verbal prompts and reminders=staff prompt client to clean, take a shower, change clothes, etc.
Housekeeping – Laundry

 

One (or a combination) of the following:

-Visual oversight = staff give organizational direction to client

– Side by side assistance=staff teach client how to complete task through role-modeling, staff carry laundry, staff work beside client to complete tasks

Benefit Coordination

 

One (or a combination) of the following to coordinate SSI, Economic Assistance, MA:

-Verbal prompts and reminders = staff remind client to complete and submit paperwork, staff are available for questions

-Visual oversight = staff are present when client fills out forms to answer questions

– Side by side assistance = client present while staff are completing paperwork, staff are to provide information to complete forms

-Totally Dependent = staff completes all paperwork without client involvement

Services Independent Haven Does NOT Provide

  1. Assistance in transferring, for example between chair and bed, chair and toilet, etc.
  2. Management of insulin injections and sliding scale adjustments
  3. 24/7 “live- in” assistance
  4. Hospice care
  5. Medicare reimbursed “skilled” home care services or physical therapy, respiratory therapy, speech therapy, occupational therapy
  6. Overnight companion/aide/private duty nursing assistant
  7. Eating assistance or supervision related to choking risk, refusal, resistance, or disagreement.
  8. Medication administration requiring two staff to physically assist or verbally cue client to take medication
  9. Medication administration if the medications are set up by someone other than our nurse, a pharmacist or a physician.
  10. Emergency use of manual restraints.
  11. Physical supervision to prevent clients from leaving the building, unattended.
  12. Regularly scheduled services between the hours of 10:00 p.m. and 6:00 a.m.
  13. Assistance to verbally cue or physically assist the client to the toilet.
  14. Assistance with activities of daily living (physically dress, groom brush teeth, brush hair, shave, nail care, or bathe a client to maintain personal cleanliness and hygiene).
  15. Scheduled daily clinical monitoring or supervision of a client’s physical condition.
  16. Home-maker services.
