Thank you for your interest in Independent Haven. We are a waivered services provider of apartment living with customized and supported living services in Hennepin and Wright Counties. We are licensed through the Minnesota Department of Human Services (245D) and the Minnesota Department of Health (Comprehensive Home Care). We accept DD, BI and CADI waivers.
We provide services in apartment buildings located in Minneapolis, Brooklyn Park and Buffalo. Clients have their own apartment.
Independent Haven customizes services based on the needs of each individual. We focus on assisting clients in developing independent living skills. Some individuals may “graduate” from Independent Haven to a more independent setting while other individuals may make Independent Haven their permanent home.
Clients’ services are paid for through waivered services. Independent Haven services per diem costs average from $120 – $175.
In addition to services paid for through waivered services, clients are responsible to pay rent and may use Social Security benefits, GRH and/or private income. Rent varies from site to site. Utilities also vary from site to site. Clients may be eligible for MSA and/or Economic Assistance to pay for food, non-food and other expenses.

This is the first call to make if you are interested in accessing county social services like: mental health case management, Minnesota health care programs, waivered in-home services, lists of ARMHS providers, lists of Adult Foster Care homes, information about emergency case management, homeless outreach services, basic needs, etc.
